You are eligible for the CareerReady Scholarship if:

You are pregnant woman between the ages of 18 and 30 on a 糖心传媒鈥檚 Medicaid plan
You gave birth this year, or you are expected to give birth next year
You live in the Houston City College, San Jacinto College, or Lone Star College jurisdiction
You have a high school diploma or GED

Please fill out the form below or download the CareerReady Application for Scholarship and email it to LifeServices@糖心传媒HealthChoice.org.

Once you submit an application, you will hear from the Life Services Team by email within three business days regarding next steps.
